This form may be used to report your Serra Club Officers' Roster for the coming Serra year to USA Council of Serra International Office in Chicago.

Please provide the Club Officers' full name and note this is annual requirement for all Serra clubs to submit the incoming club officer information, so that our records will be complete.

The USA Council requires that each Club Officer reaffirm their commitment to their Club Officer positions each year for those serving more than a one year term.
